Privacy request procedure
If you are a victim of domestic violence or other abuse, you may request that we send communications of claim related or policy related information to you by alternative means or at an alternative address. Please be advised that we have up to three (3) business days to implement your request. To provide us with alternative contact information, please write to us at:

Procedure to revoke a request
If you previously submitted a request for privacy or alternative contact information and would now like to revoke that request, write to us at the above address.
For New York Residents
New York Insurance Law Section 2612 states that no insurer, solely because an individual is or has been the victim of domestic violence, may:
a. refuse to issue or renew, deny or cancel any insurance policy;
b. demand or require a greater premium or payment from any person; or
c. designate domestic violence as a preexisting condition, for which coverage will be denied or reduced.
The fact that a person is or has been a victim of domestic violence is not a permitted underwriting criterion.
In addition, an insurer must keep confidential and shall not disclose the address and telephone number of an insured that is the victim of domestic violence, or any child residing with the victim, and the name, address, and telephone number of a person providing covered services to the victim, to a policyholder or another insured covered under the policy against whom the victim has a valid order of protection, if the victim, the victim’s legal representative, or if the victim is a child, the child’s parent or guardian, delivers to the insurer at its home office a valid order of protection.

For Illinois Residents
Illinois Insurance Law 215 ILCS 5/355b requires that any company that issues, delivers, amends, or renews an individual or group policy of accident and health insurance shall accommodate a reasonable request by a person covered by a policy issued by the company to receive communications of claim-related information from the company by alternative means or at alternative locations if the person clearly states that disclosure of all or part of the information could endanger the person. If a child is a covered person, then the right established by this section may be asserted by the child’s parent or guardian.
Except with the express consent of the person making the request, a company may not disclose to the policyholder: (1) the address, telephone number, or any other personally identifying information of the person who made the request or child for whose benefit a request was made; (2) the nature of the health care services provided; or (3) the name or address of the provider of health care services.
